Request Data

To request data from the UCI Health Enterprise Data Warehouse, visit Service Portal click on “Service Catalog”, click on the category “Reporting & Analytics”, then select “Data Request”. Please provide as many requirements and details as you can and then click the blue “Order Now” button.
